Direct Primary Care (Self-Pay Plan)

In direct primary care, members pay a recurring membership fee to be part of the practice. This is specifically for patients who do not have insurance or have a high deductible. You cannot use membership if you plan to bill insurance. In exchange for free, which is $55/month, a patient receives a pre-defined set of primary care services that includes quick access to the provider through the same or next-day appointment, including text and cell phone, basic labs, and other procedures like EKG, X-rays, etc are not included.

We accept Medicare, Medicaid, and most commercial insurance plans and we accept multiple forms of payments.

Included in the membership are as follows:

  • Sinus Infection
  • Bronchitis
  • Chest Cold
  • Sore Throat
  • Covid 19 infections
  • Flu
  • Mono
  • Yeast infections
  • Food poisoning
  • Dehydration
  • Allergies
  • Asthma
  • Ear infection
  • Sprains
  • Concussion
  • Ear wax
  • Nausea/Vomitting
  • Diarrhea
  • Women’s care

A monthly membership fee of $55 is withdrawn every 30 days and will be automatically deducted from the payment details you provided.
